New Patients

Before your visit

• Bring your photo ID and insurance card.
• Bring the shoes you wear most often, plus any braces or orthotics you use.
• Bring a list of your current medications.
• If you have had recent imaging elsewhere, bring the report if you have it.

Insurance and billing

Coverage varies by plan. Our staff can answer coverage and billing questions directly by phone at 480-831-0700. Because plan participation and out-of-pocket costs differ from plan to plan, we confirm those details with you directly rather than listing them here.

New-patient paperwork and the secure portal

After your appointment request is reviewed and accepted, Metro Foot & Ankle will send you an invitation to OnPatient. You can use the secure portal to complete your new-patient paperwork online before your visit, or complete the paperwork at the office when you arrive.

Established patients: Sign in to OnPatient to access the portal features enabled by Metro Foot & Ankle. If you need help accessing the portal, call 480-831-0700.

For your privacy, do not send symptoms, diagnoses, medical history, medications, insurance member details, records, or images through this website. Requested onboarding information can be completed securely in the OnPatient portal, or at the office. Call the office to discuss anything clinical.
